Equity Declaration - Self-Identification
This questionnaire is voluntary. The information you provide will support CEIU’s equity and human rights initiatives.
Your responses may be used to:
- Help plan equity and human rights activities and events
- Support communications with equity-deserving members
- Confirm eligibility to participate in CEIU Equity & Human Rights and Race Relations committees (national and regional)
- Confirm eligibility to hold positions on these committees
- Confirm eligibility to vote in elections for CEIU’s National Vice-President and Alternate for Human Rights and Race Relations
- Inform CEIU’s research, advocacy, programs, and strategies to better respond to members’ needs
Please note, answering, “prefer not to say”, will exclude your ability to participate in the equity election process.
All information collected will be handled with strict confidentiality and managed in accordance with CEIU’s privacy policies.
Confidentiality and Consent
- CEIU values your privacy and is firmly committed to protecting and safeguarding your personal information. You may amend, correct, or revise your information at any time.
- This information is kept confidential and collected and securely stored for planning purposes. You may request to amend it at any time. Please note that this information may be shared within different structures of the union, including staff and elected union representatives, in support of our human rights work only including event planning, organizing and communications. The sharing of members’ contact information is done in compliance with CEIU Privacy and Confidentiality policies.
- Please note: The employer and/or PSACs equity and self-identification forms do not replace this form. To self-identify as an equity-deserving member with CEIU this form must be completed.
