Useful forms for CEIU members. Some forms are for internal union work, while others are needed for actions involving the employer, such as grievance forms.
All members must sign a membership card to become a member in good standing and enjoy full union benefits.
This self-identification form has three purposes: it allows equity members to receive information about the Human Rights and Race Relations Committee’s initiatives, to participate in the committee’s activities, and to vote in the election of CEIU’s National Vice-President for Human Rights.

These forms are being offered by the Federal Public Sector Labour Relations and Employment Board as an online service and are available in PDF only.
This PSAC form allows members to request that their status as a member in good standing be maintained while they are on leave or while their union dues are being impacted by a pay issue.
